ISA Monitor: Action Logged

When I use ISA monitoring feature, I can see logged events based on defined filters. One of the column is "Action". If the filter is set to web proxy, when "Action" shows denied, does it mean that it is ISA or the destination site is denying the traffic? I understands probably what "Initiated" and "Closed" mean but am confused bout the "Denied" because I allowed all outbound traffic from the internal network just for testing, some connections are still denied. Please share your thoughts. Thanks in advance.

denied means ISA blocked the traffic.
