ISA and external H.323 videophones

I have H.323 enabled and working with Netmeeting both internally and expernally. We have a H.323 videophone that I can call on our internal network from NetMeeting. But, if I call an external H.323 videophone, I get a brief video, then it disconnects. If I look at the ISA logs, I see that the external phone opens a bunch of UDP with port numbers in the 10000 region. I notice that if I call an external netmeeting client I also see these UDP ports open. The logs show these ports as blocked as well, but Netmeeting works, the H.323 videophone does not. Any ideas?
