I've got a server publishing rule for HTTP from an external IP to an internal web server. It works and everything but when I telnet to the web server from the outside on the http port it sometimes takes forever to establish the TCP session internally often long enough to timeout most browsers. YetsSometimes its established instantenously. The same goes for SMTP and anything else published thru. The clients don't have a NAT client installed. Its not DNS or routing as near as I can tell.

Sounds like a router or ISP problem.
