I use server publishing for my sites and the external incoming request work fine. My published servers are secure nat clients with an internal DNS that forwards to one of my internet dns boxes. The snat servers can get to any domain on the internet but when I try to reach another published server by sending the request to the gateway (ISA) after it resolves the name, it seems that ISA can't forward this internal request to itself on the external interface. My workaround has been to list the domains also in my internal dns with their 192.168.x.x address. This works unless I try to use the host record of www and then it tries to connect using their external IP's again and fails. Is there a better way?

it is highly recommended to use a different name space internally and externally. Check out http://www.isaserver.org/authors/harrison/tutoials/dns-4-isa.htm
