To perform a tracert through the ISA, from an internal client PC to an external IP, does a port need to be opened?

Tracert relies on ICMP. You will need to enable and permit access by editing your ISA system policy.

Don't forget, ISA system policy is only for access to and from the ISA server itself (Local Host) so to allow tracert *through* ISA you will need to do this with a new rule in the firewall policy.
