We recently decided to change the way we are planning on using ISA.  Right now I have the network as internet --> cable modem --> Win SBS 2k3 --> ISA 2006 --> IIS on Win 2k3 Web Ed. 

The SBS and all user clients are on a .1.X network while the IIS box is on a .2.X network with the second nic on the ISA box.

The problem I am running into is that there are sites hosted on SBS and on our IIS box.  I have all the sites published through ISA, and all are accessible internally.  None of the websites are accessible externally however. 

I have tried forwarding port 80 on the SBS to the ISA server and having ISA send the original host headers, but still they are unaccessible from the outside.

Any ideas are greatly appreciated.

Most likely SBS is already using those ports. Try disabling IIS on the SBS box.
