I currently have a Media Server service running (HTTP disabled, MMS and RTSP only) on a IIS web server.  The web server sits in a DMZ that has both a PIX and an ISA firewall run in parallel, the PIX is the default gateway and the route that all inside traffic uses.  The ISA server is used solely for publishing the website to the internet only.

Problem is I can not get MMS to work.  I have set up multiple access rules and server publishing rules to no avail, the connection is either blocked by the default deny rule (all server publishing rules I make) or I get a "failed connection attempt" if using access rules.

Has anybody set this up successfully before and can provide guidance?

You need to set the ISA Firewall as the default gateway for the MMS and RTSP server.

I forgot to mention, I set a static route on the web server for the external IP address that I am testing with to point to the ISA machine, so technically it should be sending it to the right firewall.

Also, I checked the logging on the media server and there have been no connection attempts so it is not even getting through the firewall.

Yes, but set the default gateway on the published Web server to be the ISA Firewall's internal IP address.
