link translation not working consistently

We have a Sharepoint farm that's using ISA Server 2006. We have a rule in the ISA that redirects all traffic from ab.cb.de to server.com. That is,  a client enters ab.cb.de and this URL gets translated to server.com. The web pages then show links with server.com instead of ab.cb.de.  Our set-up is that any access via the Internet for ab.cb.de will be blocked and that every request for server.com goes through ISA. The issue is that sometimes the translation works and sometimes it doesn't. I don't know if it makes any difference that the web pages use frames. The left-side frame is able to translate consistently. It's the right-frame that is not behaving consistently. Also the URLs are long with lots of querystrings.

My thoughts:
1) Is there a setting on ISA that somehow limits the length of the query string or that limits the number of translations?
2) What is special about these frames that ISA refuses to translate?
3) Could there be a leak in the "tunnel" that sometimes the web pages don't go through ISA causing non-translation?

Anybody please help.
