video conferencing over IP and H.323 gatekeeper setups

We have a video conference server both running on ISDN and IP connections using H.323 protocol and H.263 for video coding. It's behind an ISA server and no problem with ISDN connections, but we cannot be connected via IP and also no remote site can be connected to us.
Besides protocol rules, do I need to run the H.323 gatekeeper? If so, can anybody answer the questions below as I'm not so familiar to H.323 Gatekeeper setups?

* On H.323 gatekeeper--> call routing menu, what does the destination stand for? Is it the remote site that we will place a call to or is it our video conference server where all incoming calls will be routed to? What type should I select ( gatekeeper, proxy, ils)??

* On IP routing menu, what does the IP Pattern address stand for?
