Setup is as follows: Windows 2003 standard server ISA 2004 standard
This box only serves as a firewall/proxy for my internal network.
Logged on to the console or via terminal services showed the same issue.
I even tried a reboot of the server with no success, same problem.
Then I tried a repair of ISA but that one bounced also. I had to remove ISA completely and reinstall before I got it to work again. Luckily I had a backup of my config, now I know why that's important ;-)
So all is working again, but I don't know the root cause.
ISA Server Error - Refresh failed ISA2004, might still relate somehow to dcom authentication level. See my post about the same error I had with ISA2000
ISA 2000 I did have the same monitoring > alerts > refresh failed error, and just today I found a nice fix that worked with my servers:
I figured out that this problem is due to dcom configuration and authentication level: Run dcomcnfg.exe > applications tab > FPC class (1) and (2), properties, general tab > change authentication level from default to connection > alerts will work with local session as well as remote session. Im not sure if you can change this level back to default, but I think this is better workaround than reinstalling isaserver (that didnt work for me, had still the same error)
