If you are using route relationship....
Set up the server publishing rule as you would normally
From the client perspective you connect to the actuall server not the published address on the ISA server. ISA will enforce the publishing rule for the traffic(it looks wierd but it works)
You might also have to change how the traffic appears coming from ISA to the published server depending on how your network is setup.(This is located in the published rule on the To tab)

The problem I was having was I was assuming that the client needs to connect to the published IP on the ISA server, this works great for a NAT relationship.

The is exactly what happened. I finally got it working. In my case, I had to change how the traffic appears coming from ISA to the published server instead of it coming from the published server which is not the default when you create the rule. Once I did that, it worked like a charm. Thanks for writing.

Now, if I only had a solution for publishing 992 and redirecting to port 23 on the ISA server. Is this possible?

yes it is possible
Open up the Server publishing rule and go to the traffic tab and select ports. From here you can select a different port to send traffic to on the published server

great. unfortunately, I need to also host a certificate which I offload at the ISA server 992->23, which I doubt I can do. I believe ISA can only host SSL certificates for https traffic.

Hi R,

That is correct. The ISA firewall will not terminate TLS connections for anything other than Web protocols (HTTP).
