I have read all previous posts related to this error and my situation doesn't seem to fit so I am starting a new thread.  I have created a secured web publishing rule using RADIUS to authenticate.  The IAS server is reporting successful authentcation.

Server TO: portal.tc3health.com
Public Name: Portal.tc3health.com
Paths External: <same as internal>
Paths Internal: /invoicing
Users: All Authenticated Users
Networks: External
HTTP: Disabled
HTTPS: 443
Certificate: portal.tc3health.com
Auth Methods: RADIUS
Always Authenticate: No
Domain for authentication: corp.tc3health.com

Since I only have one DNS server that is configured to resolve portal.tc3health.com to the public IP, I have configured a host file on the ISA server to point portal.tc3health.com to the internal address of the web server.  My PING tests verify that ISA server is, in fact, resolving to the private IP.

Does anyone have any words of wisdom that could assist me with this issue?

What authentication method you got set on the website i.e. Intregrated, Basic, etc. Also review the paths section. I had a problem but i was doing HTTPS--->HTTP briding.

Remember RADIUS uses basic authentication. Also make sure that you enable it to forward the username and password to the webserver - i forget the exact name of the tick box.

Also with RADIUS the username should be entered in the domain\username format.

I only had Anonymous logons enabled for the website.  I did not know that Basic authentication also needed to be check, but it still doesn't work.  I get the same result.
I cannot find any "tick box" where I can specify that the RADIUS server will forward the username and password to the webserver.  I would expect that to be automatic.  Finally, I am aware that the RADIUS server is expecting domain\username format.  Do you or anyone else have any other ideas.

So the problem ended up having to deal with Paths after all.

I had my internal path setup as /invoicing.  I changed it to /invoicing/* and it works!
