I recently setup 2 ISA Firewalls on two separate ISP internet connections into a client's network.  I setup two mx records on the internet to allow fault tolerance for their incoming email.  But I needed a way to setup fault tolerance for the outgoing email using the smart host.  I found a great article on Microsoft Technet which describes how to do this.  You can either setups round robin/load balanced smart hosts or fault tolerance.  Read the article at:   http://www.microsoft.com/technet/prodtechnol/exchange/2000/deploy/frtfytrn.mspx

To put it simply:

To load balance smart hosts simply add additional smart hosts in the SMTP Connector or Virtual Host separated by semi-colons:

[ip address];[ip address]

If you want to setup fault tolerance so the second smart host is only used if the first goes down you need to setup mx records in your dns as follows:

If they don't already exist create Host records in your internal DNS for each of your SMTP Relays.

Setup mx records for a bogus host (i.e. extmail.domain.com) for each of your SMTP Relays.  Set the preference value for your primary SMTP Relay to the lowest number and increase the value for each additional SMTP Relay mx record.

Then just enter the bogus host name as your smart host on your exchange server(s).
