I have a 3 legged ISA 2004 SE setup.  The external interface is NAT'd to the DMZ and internal.  I am routing between the DMZ and the internal network.  On the DMZ, I have an FTP server.  I have a rule that permits the traffic from the internal networks to the DMZ.  Periodically, FTP traffic get's denied with no Rule listed.  The period of time that FTP is denied is usually less than 10 seconds (I monitor port 21 from inside the ISA Server every 5 seconds and email on 2 failures and rarely get the email).
