How does the rule created in your example differ from the "Default rule" that is created when you turn on caching? The "To" network includes "All Networks", would this not also include External?

I've had a mixed set of bags with caching.  I've found some sites don't cache very cleanly. For example, Yahoo.  I had users going to Yahoo Finance and seeing "Hello" UserA when they certainly were not UserA.  I've had to create my own set of "Don't Cache" sites anytime these problems bubble up to my bucket.

What scares me is that I have seen some sites that generate what appear to ISA as static htm pages but are in fact generated dynamically.  I found this on one particular employee self-service website where users authenticated and then suddenly were seeing someone else's salary.  Not good.

I'm almost to the point of disabling caching altogether.  I can't decide where is the breakpoint? I have two active DS-3 internet feeds for around 4,000 concurrent users.  How much benefit (versus the risk) with caching am I getting?

You can fix that problem by configuring the rule to not cache content that requires authentication.
