I've never tried the MySQL setup so can't speak from expereince.. but...
Did step 1 about creating the MySQL database work?
Did step 2 about creating a mySQL System DSN work when tested?
Did step 3 about creating an access system DSN appear to work? Can you test it?
Did any of step 4 work when telling ISa to log to a database instead of to a logfile?
Does ANYTHING get logged to the database?

I don't know how to trouble-shoot any of the steps but at least we can confirm WHERE the problem is happening.

I'm assuming it used to log to file OK?
Nothing at all appears in te event logs?
Any onscreen errors within the ISA MMC console?
Can you get it to log to an actualy Access database?
Can you/have you tried logging to an MSSQL database as a test? perhaps an SQL Server express 2005 installation.
Can you log in to the MySQL database over the network using teh same credentials and perform "select * from" type queries agaisnt the database... ensuring user/pass are correct.

Generally if ISA cant find the log device (being it log files or database) the services refuse to start... you are restarting the services aren't you? Do they start?
