Does anyone have any suggestions regarding how to setup a RPC over HTTPS publishing rule? Should I create a web farm that includes my back end exchange fornt ends or should I point it to an NLB address for the exchange environment?

Matt,

Assuming you have multiple front-end.CAS servers, you could publish to a server farm within ISA rather than NLB, i.e. let ISA handle the inbound traffic rather than NLB...

This is generally good advice, but you may need to consider using NLB if you plan for the CAS servers to host legacy non-web services like POP3/IMAP.
