This might be a stupid Q, but for No Q's asked is a stupid Q.

How do I know forsure that ISA is caching my content.

I have a rule set to cache All Sites to Internet - Internet is my External Network as I didn't use the Default External network - ??

You can monitor the ISA Server Cache performance object in System Monitor.  Or, run an ISA Server Traffic and Utilisation report.
