ISA 2004 drop allowed connections

Dear Forum,

We hawe started to test ISA 2004 final.
The Upgrade from 2000 was a complete mess, with rules imported but with stupid content (like a rule was allowing outgoing, ports 0-65535 ) .

We hawe new rules from skratch, but ISA is Still randomly dropping allowed connections without logging any connection limit reached.

Also, it is randomly dropping Internal connections from 192.168.0.x IP's targeting to 192.168.0.1:25

Some log examples:

What access rules have you configured on the ISA firewall?
