My network is in the 10.x range NAT'd to a "public" address on a 164.x WAN. I have a segment of my NT domain (with a bdc) on a different WAN (150.x). Whenever I put my test ISA server in, this remote segment looses NTLM (netbios) connection. I checked the logs, and saw that ports 138 and 139 were being blocked.
I added a Packet Filter to allow "all Netbios" to my PDC. This didn't help. If I turn Packet Filtering off before putting the ISA server in place, nothing moves thru . . . I think that routing is turned off by default in this case. If I turn Packet Filtering off after the server is in place connectivity is restored, but I think this is due to the ARP cache still contains routing information, but that would probably end when the ARP ttl runs out.

How can I have Packet Filtering on, and allow netbios ports thru?

Dumb question but are you setting up your packet filters to allow "all ports" for remote computers to port 138 and 139 on the local computer? This tripped me up for a while as the remote computers usually send their requests from a random port number (e.g. something like 10080 or similar) to a specific port number, so you packet filter needs to reflect this.
